Our contact form is intentionally limited: we collect only your name, email, phone number, contact preference, and any general scheduling notes you choose to include. Please do not submit health information, diagnoses, or clinical details through this website's contact form. Any clinical information is collected securely, after your first contact with our office, through HIPAA-compliant channels.

How We Use Your Information

Information submitted through this website is used only to respond to your inquiry and schedule your free consultation. Clinical information collected once you become a client is used solely for treatment, coordination of care (with your consent), billing, and legally required record-keeping.

Information Sharing

We do not sell, rent, or share your personal or health information with third parties for marketing purposes. Information may be shared with service providers who help us operate our practice — such as our electronic health record system, scheduling platform, or payment processor — each of whom is bound by HIPAA-compliant confidentiality agreements (Business Associate Agreements). We may also disclose information when required by law, such as in cases of imminent risk of harm.

Telehealth Privacy

Telehealth sessions are conducted through a HIPAA-compliant video platform. We recommend joining sessions from a private location and using headphones where possible, to help protect your own privacy on your end of the call.

Minors' Privacy

For clients under 18, a parent or legal guardian must provide consent for treatment. Certain disclosures to parents/guardians may be limited by Florida law depending on the minor's age and the nature of treatment — this will be discussed clearly at the start of care.

Data Retention

Clinical records are retained in accordance with Florida Board of Clinical Social Work, Marriage & Family Therapy and Mental Health Counseling requirements. Website contact form submissions are retained only as long as needed to respond to your inquiry.

You may request access to, correction of, or a copy of your records, or ask questions about this policy, by contacting our office directly at (727) 618-0331. You also have the right to file a complaint with the U.S. Department of Health & Human Services Office for Civil Rights at no cost and without retaliation.

Text Message (SMS) Communications

If you opt in to receive appointment reminders or scheduling updates by text message, standard message and data rates may apply, and message frequency will vary based on your upcoming appointments. Reply STOP at any time to opt out, or reply HELP for assistance. Consent to receive texts is never a condition of receiving care. Text messages are used only for scheduling purposes — never for sharing clinical details.
